### This page is information, not medical advice

## What Metformin Is Actually Registered For Here

Metformin hydrochloride is sold in the Philippines as a prescription medicine under several brand names. The Watsons Philippines metformin product pages, checked in August 2026, mark every listing prescription required.

The labelled indication is type 2 diabetes mellitus. Depending on the specific product, the label covers use as monotherapy or alongside other glucose-lowering agents in adults, and in some presentations in adolescents. Metformin labels do not list weight reduction, obesity or appetite control as an indication.

**So weight use is off-label.** What off-label means is that the regulator has not evaluated the drug for that purpose, the manufacturer cannot promote it for that purpose, and the responsibility sits with the prescriber rather than being backed by a label. A drug being on the shelf is not the same as a drug being approved for a given use.

## What The Evidence Actually Supports

The best long-run data on metformin and body weight does not come from a weight-loss trial at all. It comes from the Diabetes Prevention Program and its follow-up study, which enrolled adults with prediabetes and tracked them for years. The weight figures were collected without anyone trying to make a weight drug look good.

Diabetes Prevention Program and DPPOS, metformin arm, published outcomes

| Measure | Metformin | Placebo |
| --- | --- | --- |
| Weight change, DPP double-blind phase | About 2.1% of body weight | About 0.02% |
| Approximate absolute loss at year 1 | Around 2.7 kg | Negligible |
| Long-term, out to about 10 years | About 2.0%, roughly 1.9 kg | About 0.2% |
| Highly adherent participants | About 3.5%, roughly 3.1 kg | Not applicable |
| Gastrointestinal symptoms reported | About 28% | About 16% |

That table is the centre of this page. In the Diabetes Prevention Program, metformin produced a real, durable weight reduction, and a small one. The highly adherent subgroup, meaning people who took it consistently for years, reached about three and a half percent.

Caution

### A different category from the injectables

There is a second caveat in the other direction. The DPP population was people with prediabetes, and in that population metformin also reduced progression to diabetes. For a patient whose fasting glucose has started drifting, weight is not the only outcome on the table.

## What It Costs in Philippine Pharmacies

The figures below are per-tablet prices read from Watsons Philippines product pages in August 2026, all prescription required and all flagged VAT exempt on the listing page.

Metformin 500mg, per tablet

| Product | Form | Per tablet | Source |
| --- | --- | --- | --- |
| Watsons Generics metformin | Immediate release | ₱4.00 | Watsons PH product page, August 2026 |
| RiteMED metformin | Immediate release | ₱4.25 | Watsons PH product page, August 2026 |
| Glumet XR | Extended release | ₱14.25 | Watsons PH product page, August 2026 |
| Glucophage XR | Extended release | ₱18.50 | Watsons PH product page, August 2026 |
| Glucophage | Immediate release | ₱19.00 | Watsons PH product page, August 2026 |

Per-tablet listings only, from one retailer, dated. They are not a dosing recommendation and not a quote, and pharmacy pricing moves.

One thing falls out of that table: on the Watsons listings, branded Glucophage costs between four and five times the plain generic per tablet for the same molecule at the same strength, which is a larger gap than many patients expect and worth raising with the prescriber.

## The Gastrointestinal Reality, And What XR Is For

Metformin has one dominant tolerability story: nausea, loose stools, abdominal cramping and a metallic taste, most noticeable when the drug is started.

The Diabetes Prevention Program quantified it in a population that was otherwise well. Gastrointestinal symptoms were reported by about 28 percent of metformin participants against about 16 percent of those on placebo. Symptoms specifically attributed to the study medication ran at 9.5 percent against 1.1 percent. Those rates declined across the trial, and by years six through nine the metformin and placebo groups looked similar.

Extended-release formulations release the drug more gradually. The Philippine market carries several, including the Glumet XR and Glucophage XR listings above.

Note

### Where extended-release is a genuine decision

Beyond the gut, the prescribing discussion covers kidney function, alcohol intake, the lactic acidosis question, long-term vitamin B12 status, and holding the drug around iodinated contrast imaging. Those are screening and monitoring questions, and they are a large part of why this molecule is prescription controlled despite its low per-tablet price.

## What Metformin Is Not

A short section, because clearing away the wrong expectations is most of the value this page can offer.

- It is not a GLP-1 drug and does not work through the same mechanism, and the [GLP-1 side-effect profile](https://www.philippinespeptides.com/guides/glp1-side-effects) is a different profile from metformin's.

- It is not an over-the-counter product in the Philippines, however low the per-tablet price.

- It is not a substitute for the diet and activity side. In the Diabetes Prevention Program, the lifestyle intervention arm lost more weight than the metformin arm.

- It is not a research compound and it is not interchangeable with one. A registered prescription tablet and an unregistered research peptide are different categories of thing.

## Taking This To A Philippine Doctor

Because metformin is prescription only and its weight use is off-label, the legitimate route runs through a licensed Philippine physician. That consultation goes better with the right material:

- The actual target, stated as a number and a timeframe, so the doctor can say whether metformin's trial results are anywhere near it.

- Recent bloods, if available. Fasting glucose, HbA1c, lipids, and renal function are the ones that shape this decision.

- Anything in the PCOS or insulin-resistance picture: cycle irregularity, skin changes, family history of type 2 diabetes.

- The budget. It changes the shortlist, and a physician who knows it can plan a sequence rather than a one-off.

- Every supplement and medicine already being taken, including anything bought online.

Caution

### Prescription medicines need a prescription

Nothing on this page is medical advice, a diagnosis, a dose, or a recommendation to take any medicine. Prices are market observations from a named retailer on the month stated and will move, and prescribing belongs with a licensed Philippine physician.

## FAQ

### Is metformin approved for weight loss in the Philippines?

No. Metformin’s labelled indication is type 2 diabetes mellitus, and it is sold in the Philippines as a prescription medicine. Use of it purely to reduce body weight in a person without diabetes is off-label, which means the prescriber is going outside the labelled indication on their own clinical judgement. That is not the same as approval, and prescribing belongs with a licensed Philippine physician.

### How much weight did people lose on metformin in trials?

Modest amounts. In the Diabetes Prevention Program, a trial in adults with prediabetes, participants on metformin lost about 2.1 percent of body weight, roughly 2.7 kg at one year, and in the long-term follow-up study that settled at about 2.0 percent, close to 1.9 kg, held over as long as ten years. Highly adherent participants reached about 3.5 percent. In the same programme, gastrointestinal symptoms were reported by about 28 percent of metformin participants against about 16 percent on placebo.

### How much does metformin cost in Philippine pharmacies?

Watsons Philippines’ product pages, checked in August 2026, list RiteMED metformin 500mg at PHP 4.25 a tablet, Watsons Generics metformin 500mg at PHP 4.00 a tablet and branded Glucophage 500mg at PHP 19.00 a tablet, all prescription only and flagged VAT exempt.

### Do the stomach side effects go away?

In the Diabetes Prevention Program, gastrointestinal symptoms were reported by about 28 percent of metformin participants against about 16 percent on placebo, and symptoms attributed to the study drug ran at 9.5 percent against 1.1 percent. Those rates declined over the trial and by years six to nine the two groups looked similar. Extended-release formulations exist partly for people in whom the symptoms do not fade, and any switch is a decision for the prescriber.
