# Xenical and Orlistat in the Philippines

Philippines Peptides editorial desk Updated 1 October 2026 12 min read

Short answer

Orlistat 120 mg, sold as Xenical, is a registered prescription medicine in the Philippines, checked August 2026. It works in the gut, blocking the enzymes that digest dietary fat, and its effect is modest next to the GLP-1 injectables. The 60 mg Alli product is not registered here, and prescribing belongs with a licensed Philippine physician.

Video guide: Diet Pills and Slimming Injections in the Philippines (2:07)

Searching for diet pills or slimming injections in the Philippines? Here is what is registered and what it costs. Xenical is orlistat, registered and prescription only. Watsons listed it at ₱51.25 a capsule in August. Alli, the over-the-counter version sold abroad, is not registered here. Contrave shows no evidence of Philippine registration either. Duromine is phentermine, a stimulant appetite suppressant. It is registered with FDA Philippines, prescription only, and labelled for short-term use. Its price? No Philippine chain pharmacy publishes one online. Slimming supplements are food supplements, not drugs, and FDA Philippines keeps issuing advisories against unregistered ones. Slimming injections usually means prescription GLP-1 pens. In September, the lowest Ozempic pen listing was ₱7,599. A doctors' group has warned against compounded, unregistered versions. Under RA 9711, a drug must be registered before it is sold. Prescribing belongs with a licensed Philippine physician. These molecules are also sold as research vials. Not the same product: a research vial is not approved for human use and is sold for research only. Our partner and supplier, Primara Labs, lists 15 mg research vials of semaglutide, tirzepatide and retatrutide, from ₱1,890. Per mg, a Mounjaro 15 mg pen works out to about ₱498. The tirzepatide vial is about ₱126. Every order includes bacteriostatic water, syringes and alcohol pads. Metro Manila orders before noon arrive the same day, with a 60-day money-back guarantee. Read the full diet pills guide on Philippines Peptides, then see the GLP-1 vials at Primara Labs and order on WhatsApp.

Orlistat occupies an odd position in the Philippines. It is registered, it is sold per capsule at Watsons, and its trial results are modest next to the GLP-1 injectables. Reading it honestly means holding both facts at once. For the whole category, the [diet pills filing](https://www.philippinespeptides.com/guides/diet-pills-philippines) sets out what else is on Philippine shelves.

## What Orlistat Actually Does

Most other weight-loss drugs act on appetite. Phentermine, sold here as Duromine, works on central noradrenergic signalling. The GLP-1 receptor agonists work through gut hormone signalling that reaches the brain and slows gastric emptying. Orlistat does none of that. It acts inside the gastrointestinal tract and does not act on hunger.

What it does is inhibit gastric and pancreatic lipase, the enzymes that split dietary triglycerides into absorbable free fatty acids and monoglycerides. An orlistat molecule binds covalently to the active serine site on those enzymes and takes them out of service. Fat that is never split is never absorbed across the intestinal wall. The Xenical label states that at its recommended dose, orlistat inhibits dietary fat absorption by approximately 30%.

Three consequences follow directly from that mechanism, and they explain almost everything else about the drug.

**The effect depends on the fat in the meal.** A low-fat meal leaves little for orlistat to block, so the calories blocked can be modest even when the drug is taken exactly as directed.

**The blocked fat has to leave the body.** Undigested triglyceride passes into the colon and exits as oil. That is not a side effect in the usual sense of an off-target reaction. It is the mechanism, observed from the other end.

**Anything that rides in on dietary fat is affected too.** Vitamins A, D, E and K are fat soluble, and reducing fat absorption reduces theirs. That is covered further down.

## What Is Actually Registered in the Philippines

Orlistat 120mg is registered with FDA Philippines and dispensed on prescription. The Watsons Philippines product pages, checked in August 2026, list it in three forms: branded Xenical, the chain generic sold as Watsons Generics orlistat, and Lesofat. All three are the same molecule at the same strength, and all three were sold per piece.

Alli is different, and this is the most misunderstood point in the category. Alli is orlistat 60mg, half the prescription strength, approved as an over-the-counter product in the United States. It has no Philippine registration, and no Philippine pharmacy stocks it lawfully. Under RA 9711 a drug must be registered with FDA Philippines before it is imported, sold or distributed; the detail is in the [regulation hub](https://www.philippinespeptides.com/legal).

### Prescription status is not a formality

## Verified Pharmacy Pricing

The figures below were read from Watsons Philippines product pages in August 2026. Philippine pharmacy pricing moves, and branch pricing can differ from online listings, so these are dated reference points rather than quotes.

Orlistat 120mg, Watsons Philippines listed prices

| Product | Format | Listed price | Per capsule | Source |
| --- | --- | --- | --- | --- |
| Xenical (Roche) | Sold per piece | ₱51.25 | ₱51.25 | Watsons PH product page, August 2026 |
| Lesofat | Sold per piece | ₱35.25 | ₱35.25 | Watsons PH product page, August 2026 |
| Watsons Generics orlistat | Sold per piece | ₱24.68 | ₱24.68 | Watsons PH product page, August 2026 |
| Watsons Generics orlistat | 8+2 capsule pack | ₱282.00 | ₱28.20 | Watsons PH product page, August 2026 |

Two things in that table are worth pausing on. The first is the spread: branded Xenical is roughly double the chain generic for the same 120mg of the same molecule. The second is that the 8+2 pack works out dearer per capsule than the same generic sold loose, which is the opposite of what a shopper reasonably assumes a multipack does. The per-capsule figure is the one to compare.

Mercury Drug publishes no medicine prices online, so Mercury pricing has to be confirmed at a branch or by phone, and a precise Mercury peso figure quoted on a blog is a figure nobody could verify online.

## Alli vs Xenical

The two products are the same drug at different strengths, sold under different regulatory regimes. Alli is orlistat 60mg, sold over the counter overseas. Xenical is orlistat 120mg, prescription-only here.

| | Alli | Xenical and PH generics |
| --- | --- | --- |
| Strength | 60mg | 120mg |
| PH FDA registration | None | Registered |
| Available in PH pharmacies | No | Yes, on prescription |
| Watsons PH listing, August 2026 | Not listed | ₱24.68 to ₱51.25 per capsule, Watsons PH product pages, August 2026 |

Because Alli has no Philippine registration, a parcel of it is an unregistered drug product, and there is no local recourse if a shipment is detained or if what arrives is not what was ordered. The registered 120mg product is available on prescription from Philippine pharmacies.

## The Honest Efficacy Number

Orlistat works, and its published figures are modest. They are worth reading plainly.

The four-year XENDOS trial found the orlistat group down 10.6kg at one year against 6.2kg on placebo, with both groups on the same lifestyle programme. By year four the two arms had converged to 5.8kg against 3.0kg. XENDOS also recorded a 37.3% relative reduction in the incidence of type 2 diabetes over four years, arguably the strongest result on orlistat's record.

Reported trial outcomes, drug against placebo. Different trials, different populations, not a head-to-head comparison.

| Compound | Trial | Duration | Reported result |
| --- | --- | --- | --- |
| Orlistat 120mg | XENDOS | 1 year | 10.6kg vs 6.2kg placebo |
| Orlistat 120mg | XENDOS | 4 years | 5.8kg vs 3.0kg placebo |
| Semaglutide 2.4mg | STEP 1, phase 3 | 68 weeks | 14.9% vs 2.4% placebo |
| Tirzepatide 5 to 15mg | SURMOUNT-1, phase 3 | 72 weeks | 15.0% to 20.9% across doses vs 3.1% placebo |

These are separate trials with different populations, endpoints and eras of trial design. Reading them side by side gives the order of magnitude, not a like-for-like ranking. XENDOS reports kilograms and the STEP 1 and SURMOUNT-1 rows report percent of body weight.

The gap is not subtle. In XENDOS, orlistat delivered a few kilograms more than placebo on top of the same lifestyle programme. STEP 1 and SURMOUNT-1 reported percentage losses in the teens and low twenties. They are not near substitutes for one another. What orlistat has going for it is registration, a long safety record and a mechanism confined to the gut. Branded pen prices are in the [GLP-1 price tracker](https://www.philippinespeptides.com/prices).

## The Fat-Soluble Vitamin Problem

This is the one genuinely avoidable risk in taking orlistat.

Vitamins A, D, E and K are absorbed alongside dietary fat. Block a portion of fat absorption and theirs is reduced too. The Xenical label states plainly that the drug has been shown to reduce the absorption of some fat-soluble vitamins and beta-carotene, and it recommends a multivitamin containing those vitamins. How to fit that supplement around the medicine is a question for the prescriber or pharmacist.

Caution

### Vitamin K and warfarin

Longer courses raise the stakes. A six-week course and a two-year course are different positions, and vitamin D status in particular is worth raising with the prescriber if the course is going to be long. Side effects across the wider weight-medicine category are covered in the [side effects filing](https://www.philippinespeptides.com/guides/glp1-side-effects).

## Side Effects, and Why Adherence Matters

Orlistat's main side effects are gastrointestinal, and they follow from the mechanism.

The Xenical label reports, for year one of treatment, oily spotting in 26.6% of patients, flatus with discharge in 23.9%, faecal urgency in 22.1%, fatty or oily stool in 20.0% and oily evacuation in 11.9%. The label characterises these as generally mild and transient, decreasing in the second year of treatment. These are not organ-level harms. They can still be a serious practical problem for someone with a long commute or fixed break times.

The effects scale with the fat content of the meal, so the drug gives an immediate feedback signal about the previous meal. The trial figures above come from people who kept taking the drug as directed. Whether the side effects fit a patient's work, commute and diet is a real part of the decision, and it belongs in the conversation with the physician alongside the clinical questions.

## FAQ

### How much does Xenical cost in the Philippines?

Watsons Philippines’ product pages, checked in August 2026, list branded Xenical orlistat 120mg at PHP 51.25 per capsule sold per piece, Watsons Generics orlistat 120mg at PHP 24.68 per capsule and Lesofat orlistat 120mg at PHP 35.25 per capsule. Mercury Drug publishes no medicine prices online, so branch pricing has to be confirmed in person or by phone. Prices move, so these are dated reference points.

### Is a prescription needed for orlistat in the Philippines?

Yes. Orlistat 120mg, whether branded as Xenical or sold as a generic, is a prescription medicine in the Philippines and pharmacies dispense it against a valid prescription. Listings that sell it loose online without one are operating outside that rule. Whether orlistat suits a given patient is a decision for a licensed Philippine physician who can see the weight history, other medicines, and gallbladder and kidney status.

### Is Alli available in the Philippines?

Not as a registered product. Alli is the 60mg over-the-counter orlistat sold in the United States and parts of Europe, and it has no Philippine FDA registration, so no local pharmacy stocks it lawfully. Under RA 9711 a drug must be registered with FDA Philippines before it is imported, sold or distributed. The 120mg prescription product is the registered orlistat here.

### How much weight did people lose on orlistat in trials?

Less than most people expect. In the four-year XENDOS trial, the orlistat group was down 10.6kg at one year against 6.2kg on placebo, with both groups on the same lifestyle programme, and by year four the gap had narrowed to 5.8kg against 3.0kg. On side effects, the Xenical label reports oily spotting in 26.6% of patients in year one, flatus with discharge in 23.9% and faecal urgency in 22.1%. For comparison, STEP 1 and SURMOUNT-1, the phase 3 trials of semaglutide and tirzepatide, reported mean losses in the teens and low twenties as a percentage of body weight.

### Why does orlistat cause oily stools and urgency?

Because the drug works inside the gut rather than in the brain. The Xenical label states that orlistat inhibits dietary fat absorption by approximately 30% at its recommended dose, by blocking gastric and pancreatic lipase. That unabsorbed fat still has to leave the body. The Xenical label reports oily spotting in 26.6% of patients in year one, flatus with discharge in 23.9%, faecal urgency in 22.1% and fatty or oily stool in 20.0%. The effects track the fat content of the diet.

### Does orlistat interfere with vitamins?

It reduces absorption of some fat-soluble vitamins and beta-carotene, which is a direct consequence of blocking fat absorption. The Xenical label recommends a multivitamin containing vitamins A, D, E and K plus beta-carotene for people taking it, and how to fit that around the medicine is a question for the prescriber or pharmacist.
