The strongest evidence found for NAD+ comes from small human studies, and every finding below carries its evidence level.

## What is NAD+

NAD+ (nicotinamide adenine dinucleotide) is a coenzyme central to cellular energy metabolism and redox balance, and a cofactor for enzymes involved in DNA repair and metabolism. Browse the full [peptide library](https://www.philippinespeptides.com/peptides) for related compounds.

NAD+ levels fall with age, which is the rationale researchers give for supplementation. When NAD+ is infused into people it is cleared from plasma quickly and broken down by NAD-consuming enzymes, so how much reaches cells intact is unclear.

The human evidence for NAD+ itself comes from small randomised trials, none labelled phase 2 or 3, in adults with ischaemic heart failure and in healthy adults measuring blood NAD levels. Trials of the oral precursors NMN and NR are a separate body of evidence and are not counted here.

## How it works

Energy metabolism: NAD+ carries electrons in the reactions cells use to produce energy and maintain redox balance. This is established biochemistry, not a finding about supplementation.

Enzyme cofactor: NAD+ is used up by enzymes involved in DNA repair and metabolism, including PARPs and sirtuins.

Breakdown: infused NAD+ is cleared from plasma quickly and broken down by NAD-consuming enzymes, so how much reaches cells intact is unclear. In a small phase 0/1b randomised trial in healthy adults aged 45 to 75, an oral NAD+ formulation raised whole-blood NAD inside cells against placebo.

## Benefits studied

- Heart pumping function in heart failure: Small human studies A single-centre randomised trial of 180 adults with ischaemic heart failure, phase not stated, measured ejection fraction of 45.44% on NAD+ against 42.44% on placebo at one month; clinical events did not differ significantly. - Blood NAD levels: Small human studies A phase 0/1b randomised trial in healthy adults aged 45 to 75 found an oral NAD+ formulation raised whole-blood NAD by 53% against placebo after five days; no clinical endpoint survived correction for multiple testing. [Source: GeroScience 2026](https://doi.org/10.1007/s11357-026-02399-1)

- Heart muscle after blocked blood flow: Animal and cell In an animal study, pigs given NAD+ before blood flow was restored after a 90-minute coronary blockage had less heart-muscle death and better recovery of heart function than pigs given saline. - Brain injury after stroke: Animal and cell In an animal study of temporary stroke in mice, NAD+ reduced the damaged area, brain swelling and neurological deficits. [Source: Front Pharmacol 2023](https://doi.org/10.3389/fphar.2023.1096533)

## Side effects reported

No human trial in the datasets reports adverse-event rates for NAD+, whose human evidence is limited to small human studies (3 completed randomised trials identified). Without controlled data, its safety in people is unknown rather than established.

## NAD+ in the Philippines

This site has not published an FDA Philippines registration result for NAD+: the 23 September 2026 portal search behind the regulation hub did not include the name, and the store's own search the same day timed out. What FDA Philippines has published about NAD+ concerns a different kind of product. FDA Philippines warned the public against buying or consuming an unregistered liposomal NAD+ food supplement in capsules, for which no Certificate of Product Registration had been issued (FDA Advisory No. 2025-0084, 30 January 2025).

That distinction matters, because NAD+ reaches Philippine buyers in more than one form. A capsule sold as a food supplement is one; the research vial is another. Primara Labs, our partner and supplier, lists NAD+ as "Lyophilized powder, 1000 mg / vial", a lyophilized injectable format, not a capsule. The advisory above concerns the capsule. The [NAD+ buying guide](https://www.philippinespeptides.com/guides/nad-philippines-guide) compares the routes to NAD+ in the Philippines, and the [NAD+ therapy guide](https://www.philippinespeptides.com/guides/nad-therapy-philippines-2026) covers the clinic side.

For research context, the evidence dataset counts only trials that gave NAD+ itself and leaves out precursors such as NR and NMN. On that count it finds 3 randomised human trials, none labelled phase 2 or 3, and the largest gave NAD+ intravenously to adults with ischaemic heart failure (Yu X et al. Am J Cardiovasc Drugs 2026). The heart and brain injury findings come from animals. For a mitochondria-targeted peptide with a completed phase 3 record, see [SS-31 in the Philippines](https://www.philippinespeptides.com/peptides/ss31). Neither the US FDA nor the EMA has an approval on record for NAD+ as a medicine.

## FAQ

### Has FDA Philippines issued a warning about NAD+?

Yes, about a supplement rather than a vial. FDA Advisory No. 2025-0084, 30 January 2025: FDA Philippines warned the public against buying or consuming an unregistered liposomal NAD+ food supplement in capsules, for which no Certificate of Product Registration had been issued.

### Is an NAD+ supplement the same product as an NAD+ vial?

No. A capsule sold as a food supplement and a lyophilized injectable vial are different products in different forms. Primara Labs, our partner and supplier, lists NAD+ as "Lyophilized powder, 1000 mg / vial". The FDA Philippines warning on record for NAD+ concerns an unregistered supplement capsule, not a vial.

### Can NAD+ be brought into the Philippines from abroad?

This site has no Philippine registration result on record for injectable NAD+. DOH-FDA-BOC Joint Circular No. 1, dated 22 June 2015, lets travellers bring listed FDA-regulated products into the Philippines without prior FDA clearance if the products are for personal use and within set limits. For prescription drugs, Joint Circular No. 1 sets the allowed quantity as the amount stated on a prescription that carries the physician's PRC licence number or a foreign equivalent. Joint Circular No. 1 does not say whether its prescription-drug allowance covers drugs without a Philippine CPR. No FDA circular that answers this was found. The Bureau of Customs lists pharmaceutical products as regulated imports that require a valid Certificate of Product Registration and/or an FDA License to Operate as importer. This is general information, not legal advice.

### How does NAD+ differ from NMN and NR?

NMN (nicotinamide mononucleotide) and NR (nicotinamide riboside) are precursors that cells convert into NAD+. The evidence count on this page covers trials of NAD+ itself and excludes precursor trials, so this page makes no comparison between NAD+ and its precursors.

### What evidence exists for NAD+?

The evidence dataset this site draws on (checked 2026-09-23) rates NAD+ at the level "Small human studies", with 3 completed human randomised controlled trials identified. Three randomised trials of NAD+ itself, none labelled phase 2 or 3: an intravenous trial in 180 adults with ischaemic heart failure, an oral-formulation biomarker trial (60 randomised) and an 11-person intravenous pilot. Trials of precursors such as NR and NMN are excluded. It has been studied for: Ischaemic heart failure (intravenous); blood NAD levels in healthy adults (oral and intravenous); heart and brain injury after blocked blood flow in animals.

Risk · Disclosure · Editorial Status

This page is editorial information for adults researching peptide therapy. It is not medical advice, not a prescription, and not an endorsement of any specific product, supplier, or protocol. Side effects can be serious and individual response varies. Talk to a licensed physician (ideally one with peptide-specific clinical experience) before starting any compound, adjusting a dose, or discontinuing therapy.

In the Philippines, GLP-1 medications are prescription-only under FDA Philippines regulation. Research-grade peptides occupy a regulatory gray area: import for personal research use is generally tolerated, but the Bureau of Customs reserves the right to inspect and detain shipments. Do not import for resale.
