MOTS-c Alabang: The Honest Version

The Alabang reader is nothing like the BGC one and the difference is not just age. This is household territory. Decisions get discussed with a spouse, sometimes with adult children, and frequently the person writing to me is buying on behalf of somebody else. The framing is rarely optimisation. It is usually worry.

The other thing that separates this district is that the underlying medical situation is often real. Insulin resistance and prediabetes are widespread in Manila, meals here lean heavily on white rice, and national type 2 diabetes prevalence sits around 6 to 7 percent. In a BGC condo a MOTS-c enquiry usually sits on top of an already healthy person chasing a marginal gain. In Ayala Alabang it frequently sits on top of an HbA1c that a doctor has already flagged.

That raises the stakes on the evidence question rather than lowering them, which is the opposite of how it tends to get treated.

Here is the case laid out plainly, because Alabang readers deserve it more than anyone.

None of that means MOTS-c is uninteresting. The 2015 discovery work out of USC, and the 2021 Nature Communications characterisation of it as an exercise-induced regulator of age-dependent physical decline, are legitimately fascinating pieces of mitochondrial biology. But interesting biology and a treatment are different objects, and the gap between them is exactly where a worried buyer is most likely to lose money.

If you are going ahead anyway, the Alabang handoff is its own discipline and it is unlike anything north of the river.

Ayala Alabang runs a proper gate. Riders are logged, frequently held, and depending on courier accreditation and time of day may not be permitted through at all, in which case a household member or staff collects at the guardhouse. That is a genuine constraint for temperature-sensitive parcels and the reason cold-chain deliveries into the village are more troublesome than anywhere else in the metro.

MOTS-c is the compound that shrugs it off. Sealed lyophilized powder in insulation does not care about twenty minutes at a guardhouse or an hour waiting for somebody to walk down. Filinvest and the Molito surrounds are lighter on procedure and closer to ordinary metro behaviour, and the mixed subdivision stock in between varies house by house.

The one thing worth doing is telling whoever is likely to receive it that a parcel is coming. In a household with staff, the failure mode here is not a lost box, it is a box that sits at the guardhouse because nobody knew to expect it.

Alabang has an in-district lab layer, which spares you the drive north. Asian Hospital runs outpatient labs and endocrine consults, covering the fasted insulin, HbA1c and lipid work that any metabolic experiment needs. A fasted insulin, HbA1c and lipid panel runs PHP 2,800 to 3,800; an OGTT runs PHP 1,800 to 2,500.

For a household buyer that consult is worth more than the vial by a distance, and unlike the vial it is a purchase I would defend without hesitation. It tells you what is actually happening, it gives your doctor something to work with, and it is repeatable.

South metro houses have better refrigeration than a Manila walk-up and worse control than a BGC studio, because the fridge is a shared family object with four people and a helper in and out of it all day.

Sealed vials stay dry, sealed and out of the fridge entirely. Once reconstituted, a vial needs 2 to 8 degrees and gets discarded at fourteen days, and it should be somewhere it will not be moved, borrowed or mistaken for something else. A labelled container on a middle shelf rather than the door is the whole trick, since the door is where the temperature swings most.

In practice a 10mg vial at the commonly cited 5mg dose is two injections and finishes inside a week, so the fourteen-day window rarely comes into it. Reconstitute one at a time and keep the rest dry.
