MOTS-c Metro Manila: Sourcing Map

Start with the honest part. MOTS-c is the hardest compound in this catalogue to actually get hold of inside the Philippines, and that is not a temporary stock condition. It is structural.

There is no pharmacy lane, because there is nothing to dispense. MOTS-c has no FDA Philippines registration, no licensed prescribing route, and no approved product in any jurisdiction. Mercury Drug and Watsons do not stock it and will not order it in. What they will sell you is bacteriostatic water and insulin syringes, which is a genuinely useful thing to know and also the full extent of the retail pharmacy contribution here.

Below that, the picture thins further. Retatrutide moves in volume because a lot of people want to lose weight. BPC-157 moves because a lot of people have a bad shoulder. MOTS-c has neither demand engine behind it, and suppliers stock what turns over. The practical result is that most local sellers who list MOTS-c are listing it rather than holding it, and the vial you order is being sourced after you pay rather than before.

That is the single most useful thing I can tell a Metro Manila reader about this peptide. Before you compare prices, confirm that the vial physically exists in the country. On MOTS-c, in-country stock is worth more than a discount.

MOTS-c is a genuinely unusual molecule and it deserves the interest it gets, so it is worth stating precisely what is known and precisely what is not.

It was identified in 2015 by Changhan Lee and Pinchas Cohen at the USC Leonard Davis School of Gerontology. The name unpacks as Mitochondrial Open reading frame of the 12S rRNA type-c, and the important part of that mouthful is that the peptide is encoded inside the mitochondrial genome rather than the nuclear one. That makes it one of a very small set of known mitochondria-derived peptides, and it means MOTS-c functions as a retrograde signal running from the mitochondria back to the nucleus.

The preclinical literature covers four mechanistic threads. Lee 2015 in Cell Metabolism describes AMPK activation, the cell’s master metabolic sensor, alongside effects on obesity and insulin resistance in rodent models. Kim 2018, also Cell Metabolism, describes nuclear translocation, where the peptide physically moves to the nucleus under metabolic stress and regulates gene expression there. Reynolds 2021 in Nature Communications characterises MOTS-c as an exercise-induced regulator of age-dependent physical decline in mice. Lu 2019 in Frontiers in Endocrinology covers adipose thermogenic activation, again in rodents. FOXO signalling and mitochondrial biogenesis run through the same body of work.

I labour this because MOTS-c is marketed harder than its evidence base can carry. Compare it with what sits at the other end of this site: retatrutide has Jastreboff NEJM 2023 with 24.2 percent mean weight reduction at 48 weeks in humans. MOTS-c has mice with better treadmill numbers. Both are interesting. They are not the same category of interesting, and a supplier who blurs that distinction is telling you something about the supplier.

The MOTS-c reader is not the reader for the rest of this site, and pretending otherwise would make these pages useless. Almost nobody arrives at MOTS-c first. They arrive from NAD+, or from Epitalon, or from a stack that already includes SS-31, which is to say they arrive having already spent a year in mitochondrial and longevity material.

That produces a different conversation. Nobody asks whether MOTS-c will get them shredded. They ask whether the 5mg figure has any dose-ranging basis, whether the nuclear translocation finding was in a cell line or in tissue, whether stacking it with NAD+ is mechanistically redundant. Those are better questions than I get anywhere else on this site.

The Filipino metabolic backdrop is part of why the interest exists at all. Insulin resistance and prediabetes are widespread in Manila, meals here lean heavily on white rice, and national type 2 diabetes prevalence sits around 6 to 7 percent. Add traffic and heat, which genuinely suppress how much a desk-bound professional moves, and an AMPK-adjacent compound reads as relevant on paper. Reading as relevant on paper is where that observation stops.

Now run it. The commonly cited research protocol is 5mg subcutaneous, three to five times weekly. A 10mg vial reconstituted in 2mL of bacteriostatic water yields 5mg per millilitre, which on a U-100 insulin syringe is 50 units per dose. Two doses per vial. At the low end of the frequency band, three doses a week, that is one and a half vials a week, or roughly ₱6,300 to ₱8,250 every week in compound alone. An eight-week block at three times weekly is 24 doses, which is 12 vials, which is ₱50,400 to ₱66,000 before you have bought a single syringe or run a single panel. At five times weekly it is nearly double that.

That makes MOTS-c comfortably the most expensive compound per week covered anywhere on this site, on the thinnest evidence base of any of them. Both halves of that sentence are true at once, and the reader who works it out for themselves usually reaches the same conclusion I did: this is a compound for people who have already exhausted the cheaper and better-evidenced options, not a compound to start with. MOTS-c ships as lyophilized powder, and the dry vial is the resilient state. That is the one thing this compound makes easy. A sealed dry vial in insulation tolerates a Metro Manila courier leg, an afternoon behind a guard desk, and a long queue at a village gate far better than anything that arrives already in solution.

Reconstituted is the fragile state: 2 to 8 degrees, transported in an insulated pouch, discarded at 14 days. In practice the 14-day window is rarely what binds, because a 10mg vial at 5mg a dose is finished inside a week at any protocol frequency. What binds instead is ordinary Manila household reality. Brownouts, shared fridges, condo units left unpowered while somebody is out of town, and a vial that ends up in a door shelf where the temperature swings every time somebody opens it.

Reconstitute one vial at a time and leave the rest dry. On a compound this scarce and this expensive per dose, a spoiled vial is not an inconvenience, it is a week of the protocol and several thousand pesos.
