Monthly Cost (Out of Pocket)
Each pen lasts 4 weekly injections. Monthly cost = pen price × ~1.0 (assuming weekly dosing at the labeled dose). For dose escalation, you usually need different SKUs per month.
| Protocol | Monthly PHP | Monthly USD | Annual USD |
|---|---|---|---|
| Ozempic 0.5mg weekly maintenance | ₱14,150 | $253 | $3,036 |
| Ozempic 1.0mg weekly maintenance | ₱14,800 | $264 | $3,168 |
| Wegovy 1.0mg weight-loss | ₱23,250 | $415 | $4,980 |
| Wegovy 2.4mg flagship dose | ₱27,650 | $494 | $5,928 |
| Mounjaro 10mg weekly | ₱19,600 | $350 | $4,200 |
| Mounjaro 15mg flagship dose | ₱20,350 | $364 | $4,368 |
These are out-of-pocket costs at retail Mercury Drug. PhilHealth does not cover GLP-1s for weight loss; HMOs (Maxicare, Intellicare, Medicard) cover only the diabetes indication and require pre-authorization with HbA1c documentation above 6.5%.
Pharmacy Price Variance
| Pharmacy | Ozempic 0.5mg PHP | vs Mercury Drug | Notes |
|---|---|---|---|
| Mercury Drug Salcedo | ₱13,800 | Baseline | Most reliable stock |
| Mercury Drug SM Aura | ₱13,500 | −2% | Slightly cheaper, BGC standard |
| Watsons SM Aura | ₱14,100 | +2% | Stocks GLP-1s less consistently |
| St. Luke’s BGC pharmacy | ₱15,400 | +12% | Hospital markup; convenient post-consult |
| Makati Med pharmacy | ₱14,600 | +6% | Hospital markup; reliable stock |
| Asian Hospital Alabang | ₱14,900 | +8% | Southern alternative; reliable |
| South Star Drug (BGC) | ₱14,400 | +4% | Smaller chain, occasional stock |
The pharmacy spread on a single pen is small (~12% top to bottom). The bigger variable is stock — a 12% premium at the hospital pharmacy is often worth paying when the retail counter is empty.
Hospital vs Retail Pharmacy
Hospital pharmacies(St. Luke's BGC, Makati Med, The Medical City, Asian Hospital) carry GLP-1s reliably because the prescribing endos write directly into hospital pharmacy queues. Markup runs 6–12% above Mercury Drug retail.
Retail flagships (Mercury Drug Salcedo, Glorietta, SM Aura, Market! Market!) are baseline price but go empty Friday afternoon to Monday morning.
Practical pattern — fill the first prescription at the hospital pharmacy on the day of your endo consult (no separate trip, no stockout risk). Refill at Mercury Drug flagship Monday mornings to save 6–12% per pen.
Grey-Market Alternatives (Research-Grade)
Research-grade semaglutide and tirzepatide sold by international suppliers run roughly 5–6x cheaper than the Mercury Drug pen on a cost-per-dose basis. The molecule is identical; what you lose is FDA Philippines registration, audited cold-chain, and pharmacy callback support.
| Compound (Research-Grade) | Cost per Weekly Dose USD | Brand Equivalent USD | Multiplier |
|---|---|---|---|
| Semaglutide 0.5mg/week | $10–14 | $60–66 (Ozempic) | ~5× |
| Semaglutide 1.0mg/week | $15–22 | $64–69 (Ozempic) | ~3.5× |
| Semaglutide 2.4mg/week | $22–32 | $120–127 (Wegovy) | ~5× |
| Tirzepatide 5mg/week | $14–20 | $81–87 (Mounjaro) | ~5× |
| Tirzepatide 10mg/week | $20–32 | $84–91 (Mounjaro) | ~3.5× |
| Retatrutide 4mg/week | $28–45 | Not approved (no brand) | N/A |
Grey-market pricing is highly supplier-dependent. Always require third-party COA per batch, cold-chain insulated packaging, and your own baseline labs before starting any cycle. See our PH supplier guide for the verified bridge.
My Experience
First Ozempic refill, January 17 2026, Mercury Drug Salcedo. Paid ₱14,200 (USD 254) for one Ozempic 0.5mg pen. Cash was faster than card. The pen lasts 4 weekly doses; cost-per-dose ~$63 USD. That worked out to ~$253 USD/month at maintenance.
Hospital vs retail premium, March 7 2026.Mounjaro 5mg was empty across three Mercury Drug flagships on a Sunday. Drove to Asian Hospital Alabang the next day, filled at ₱19,400 — about $24 USD more than Mercury Drug retail, but I had the pen in 22 minutes total. The hospital premium is real, and it's worth it when the retail counter is empty.
Switching to research-grade, March 2026. After the third Mounjaro stockout cycle, I switched my GLP-1 protocol to research-grade tirzepatide from a verified supplier with batch-level COA. Cost per weekly dose dropped from ~$85 USD to ~$22 USD. Same molecule. Same dose response. The trade-off is that I run my own labs at Hi-Precision quarterly (~$95 USD per panel) instead of leaning on the Mercury Drug audited cold-chain.
Annual math, my actual numbers. Branded Mounjaro 10mg at Mercury Drug = ~$4,200/year. Research-grade tirzepatide 10mg = ~$1,150/year + 4× Hi-Precision panels at ~$380/year = ~$1,530/year all-in. Difference: ~$2,670/year savings, in exchange for taking on the diligence work yourself.
